電話でのご予約は仮予約となります。確定の為、必ずメールを送っていただけますようお願い致します。

当クリニックでは、心療内科・カウンセリング・言語療法のご診察をご希望の場合、システム上、窓口でのご予約もしくはメール送付でのご予約が必須となります。その為、お電話での予約は仮予約扱いとなり、患者様からのメール送付が必要となりますことをご了承ください。

お電話にて空き状況のご確認がお済みになりましたら、2営業日以内もしくは受診前日の早い方までのメール送付が必須となります。clinic@jas.org.sg 宛に、下記事項をコピーペーストし、メールにてお送り下さい。

・患者様ご本人のお名前（ローマ字）：
・日本人会会員番号（初診の方で会員様の場合のみ）：
・カルテ番号（ブルーのカードに記載）：
・ご予約希望の日時（候補を３つほど）：
・担当先生のお名前：


メールを頂きますと2営業日以内に必ずこちらから返信をしますので、返信がない場合はメールアドレスが間違っているもしくはJunkメールに入っている可能性がありますので、ご確認下さい。

ご予約の変更、キャンセルはご予約日の24営業時間前までにemailにてご連絡下さい。 期日までにご連絡がない場合、キャンセル料が加算されますことご了承ください。

また当院はシンガポールの祝日は閉院となります。シンガポールにおいては祝日の変更や追加が行われることがございます。追加、変更のあった祝日の閉院につきましては当院から特にご案内等は行いませんので、ご自身にて随時ご確認いただきますようお願い申し上げます。

上記既定の日時までにご連絡がない場合、予約は自動的に取り消されますのでご了承ください。取り消しとなった場合、ご来院頂いても診察をお断りさせていただくことがありますこともご了承ください。お手数をおかけしますが、ご理解ご協力のほど、よろしくお願いします。
なお、紹介状をお持ちの場合は、出来る限り日本語での紹介状をご持参いただけますと幸いです。

Guidelines regarding appointments made by telephone

The clinic's appointment system requires appointments to be made over the counter or by sending us an email. Please note that appointments made by telephone are treated as provisional appointments, and that an email from the patient is required to confirm the appointment.

The email must be sent within two working days of the telephone call. If the appointment is on the next day, please send the email on the day of the call itself. Please email to clinic@jas.org.sg with the date and time of your appointment and your name. We will reply to your email within 2 working days.

If you do not receive a reply, please check your email address as it may be incorrect, or your Junk mail folder. Please note that if we do not hear from you by the above-mentioned predetermined date and time, your appointment will be voided, and we may not be able to carry out a consultation even if you arrive at the clinic on that date and time. We apologise for the inconveniences caused and sincerely thank you for your understanding and co-operation.
